KAPOSI SARCOMA IN A LUNG TRANSPLANT RECIPIENT: A CASE REPORT

TOPIC: Transplantation TYPE: Fellow Case Reports INTRODUCTION: Kaposi's sarcoma is closely associated with solid organ transplant, but remains extremely rare after lung transplantation. The course of the malignancy in setting transplant limited to experience few existing case reports (1). Herein, we describe unique biopsy-proven cutaneous a recipient. CASE PRESENTATION: We reviewed post-transplant that developed 70-year-old male who underwent bilateral for IPF July 2019. DISCUSSION: This patient had an uneventful perioperative course; standard maintenance immunosuppression regimen consisted tacrolimus, mycophenolate mofetil, and prednisone, aim tacrolimus trough levels at 10-12 ng/mL through first year. In year, three episodes minimal (A1B0) acute cellular rejection need corticotherapy. At 1-year follow-up visit, reported concern lesions on forearm neck grossly appeared as brown-discolored plaques (Figures A-D). Dermal punch biopsy revealed lesion cancerous etiology, composed elongated spindle cells – namely, sarcoma. PET scan failed show evidence any other focus infection. Meanwhile, quantitative analysis human herpesvirus 8 using real-time PCR assay demonstrated elevated viral load (4.8 × 106 DNA copies/106 cells). Immunosuppression was reduced (mycophenolate mofetil discontinued), sirolimus substituted tacrolimus. present, patient's allograft function stable, no foci have been identified. CONCLUSIONS: Although well-known entity abundantly described it rarely encountered recipients. Thus, long-term prognosis poorly defined these patients.
